Former Post Office Executive Regrets Overlooking IT Expert's Horizon Scandal Report in 2004
A former Post Office executive, David Miller, expressed regret for not reading a 2004 report that identified issues with the Horizon IT system as "clearly defective." Had he done so, he could have taken action to address the problems.

IT consultant Jason Coyne was hired by the Post Office as an expert witness to assess losses at a Lancashire sub-post office run by Julie Wolstenholme.
He identified discrepancies in the software, but was dismissed and the Post Office tried to discredit his report.
The Post Office pursued Ms. Wolstenholme for £25,000 in the civil case, but eventually settled.
Mr. Coyne claimed he was effectively fired for raising concerns about the software.
